Request an Appointment

New Patients:

All new patients are required to have a physician's referral, even if your insurance does not require it.

Please check with your insurance company prior to scheduling an appointment to insure that Dr Syna is a preferred provider and that your visit will be covered.

When scheduling your first consultation appointment please have the following information available so we can better assist you.

  • Your Name, current address, home and work phone numbers ,and your birthdate
  • The names of your referring physician and primary care physician
  • Your diagnosis
  • Your insurance providers information
  • Diagnostic Imaging (Catscan, MRI, etc) Please try to provide us with when and where these tests were performed.

Once your appointment is scheduled a new patient packet will be mailed to you that includes a registration form, medical history form, driving directions, etc. Please don't hesitate to contact us at 503.291.1422 with any additional questions.

All Patients:

In the event you need to cancel an apppointment please give us at least 24 hours notice so we have the opportunity to fill the vacancy.
