Request an Appointment
New Patients:
All new patients are required to have a physician's
referral, even if your insurance does not require it.
Please check with your insurance company prior to scheduling
an appointment to insure that Dr Syna is a preferred provider and that
your visit will be covered.
When scheduling your first consultation appointment
please have the following information available so we can better assist
you.
- Your Name, current address, home and work phone
numbers ,and your birthdate
- The names of your referring physician and primary
care physician
- Your diagnosis
- Your insurance providers information
- Diagnostic Imaging (Catscan, MRI, etc) Please try
to provide us with when and where these tests were performed.
Once your appointment is scheduled a new patient packet
will be mailed to you that includes a registration form, medical history
form, driving directions, etc. Please don't hesitate to contact us at

All Patients:
In the event you need to cancel an apppointment please
give us at least 24 hours notice so we have the opportunity to fill
the vacancy.
Please note at this time we cannot schedule appointments via E-mail
due to the sensitive nature of your personal information.
